Minimum Group Size

Minimum of 2 or 5 lives (depending on state requirements). For non-employer paid, buy up and voluntary cases there is a 50% participation for groups with 99 or fewer eligible lives. 25% (with a minimum participation of 50 employees eligible) for groups with 100 or more eligible lives. For groups with 5 or fewer employee applications, Healthpak must include TransConnect®, critical illness and accident products.
